Background

Lithospermi Radix (Zi Cao) has been used in Chinese medicine for the treatment of wounds, burns, ulcers, and skin disorders for over fifteen hundred years — first recorded in the Shen Nong Ben Cao Jing. This study investigated the mechanism by which shikonin promotes skin cell proliferation and suppresses inflammation in human dermal fibroblasts.

Methods

Human dermal fibroblasts were treated with shikonin at multiple concentrations alongside ascorbic acid as a positive control. Procollagen type I was measured in culture supernatants via ELISA. Cell proliferation was assessed alongside inflammation markers and NF-κB nuclear translocation.

Findings

Shikonin promoted cell growth in human dermal fibroblasts and inhibited NF-κB nuclear translocation through proteasome inhibition. The authors concluded that shikonin promotes wound healing through cell-growth-promoting activity and suppresses skin inflammation through proteasome inhibition — making it a potential therapeutic agent both for wound healing and for inflammatory skin conditions.

Mechanistic Detail

Boswellic acids selectively and non-competitively inhibit 5-lipoxygenase, the enzyme responsible for producing leukotrienes that drive inflammation. While witch hazel has long-standing topical use, the specific molecular mechanisms underlying its dermatological effects had not been fully characterized at the cellular level. This study investigated the standardized extract and the principal compound (hamamelitannin) in human keratinocytes challenged with an inflammatory cytokine milieu — modeling the chronic surface inflammation that contributes to compromised barrier function and accelerated aging.

Findings

Witch hazel extract inhibited the release of inflammatory mediators (IL-6, IL-17C, TSLP, CCL26, MMP-9) in a concentration-dependent fashion, with IC50 values below 25 μg/mL. The mechanism was attributed to impairment of NF-κB-driven transcription. Notably, the extract counteracted the proliferative effects of IL-4 and recovered K10, a marker of skin differentiation — meaning the herb supports normal skin tissue organization at the cellular level.
